WF64: FHA Safety Criteria

Pre-Requisites

• A. MADE Project: a configured project.

Detailed Steps

1. Navigate to Preferences in the task bar and select Project Preferences.
2. Navigate to Functional Hazard Assessment and select Safety Criteria. Click on the '+' icon to create a new Safety Criterion.
3. Provide a category ID and category name for the safety criterion.
4. Create Hazard ID, name, and description for the safety criterion. The Criticality Profile is pre-selected based on the active profile of the project. Assign a criticality from the drop-down for the hazard.
5. Decision — All Hazards defined? If no, continue creating hazards for the safety criterion (return to Step 4). If yes, proceed to Step 6.
6. Provide further information by defining safety criteria and minimum safety criteria narratives.
7. Decision — All Safety Criteria defined? If no, continue creating safety criterion relevant to the functional model (return to Step 2). If yes, proceed to Step 8.
8. All the Safety Criteria for the functional model have been created.

• Step 1: Open Project Preferences — Main menu: Preferences > Project Preferences. In the Preferences tree, Functional Hazard Assessment section with sub-nodes Assumptions, Control Measures, Failure Effects, Safety Criteria, Substantiation Actions (summary page has hyperlinks to each).
• Step 2: Create Safety Criterion — Select "Safety Criteria" node. "FHA Safety Criteria" panel: "Create and/or edit Safety Criteria that can be associated with FHA Failure Conditions" table (ID, Name, Safety Criteria) with green '+' to open "Create Safety Criteria" dialog.
• Step 3: Define Safety Criterion — "Create Safety Criteria" dialog: Category ID (e.g. "ACC"), Category Name (e.g. "Adaptive Cruise Control"), a "Hazards" sub-table (ID, Name, Description, Criticality) with '+' to add hazards, and "Safety Criteria" / "Minimum Safety Criteria" free-text fields below.
• Step 4: Define Hazard — Clicking '+' in the Hazards table opens "Create FHA Hazard" panel: Hazard ID (e.g. "ACC_01"), Hazard Name (e.g. "False Sensing"), Description (e.g. "Incorrect or unreliable sensor readings leading to improper vehicle speed adjustments."), Criticality Profile (pre-selected, e.g. "PHMT RPN Criticality"), Criticality dropdown (color-coded scale: No Effect, Minimal, Very Low, Low, Moderately Low, Moderate, Moderately High, High (selected in example), Very High). Click Create.
• Step 6: Define Safety Criteria — "Edit Safety Criteria" dialog shows the Hazards table populated (e.g. ACC_01 "False sensing", ACC_02 "System lag", ACC_03 "Unintended acceleration/deceleration") plus Safety Criteria text (e.g. "ACC system should maintain a safe and consistent following distance from the vehicle ahead, taking into account the vehicle's speed and traffic conditions") and Minimum Safety Criteria text (e.g. "ACC system must maintain a minimum following distance from the vehicle ahead, as specified by relevant safety standards and regulations"). A second example category "REPD" (Reliable and efficient power delivery) with hazards REPD_01 "Fuel Leaks", REPD_02 "Turbocharger Issues".
• Step 8: Safety Criteria created — "FHA Safety Criteria" list now shows created categories (e.g. REPD "Reliable and efficient power delivery" and ACC "Adaptive Cruise Control") with their Safety Criteria text visible. Click Apply and Close.
