3.3.5 Failure Consequences

"5. Failure Consequences" is the fifth page in the NAVSEA Road Map tabbed menu and coincides with the Effectiveness filter in the NAVSEA Road Map. This page is enabled when an existing and applicable maintenance action has been verified in the previous pages.

This page contains a list of failure consequences based on the options provided by the road map. Each failure consequence is described by its consequence, and a narrative is provided for additional user information. The user selects a radio button for the relevant failure consequence, then selects the link in the lower right corner of the Failure Consequences section to continue to the Effectiveness page.

3.3.6 Effectiveness

"6. Effectiveness" is the sixth page in the NAVSEA Road Map tab and coincides with the second step ("Is Maintenance Effective?") in the Effectiveness filter in the NAVSEA Road Map. This page contains 3 main sections to assess whether an existing and applicable maintenance action is effectively reducing the risk of a failure path:

  • Effectiveness for the Failure Consequence
  • Existing Maintenance Action (see section 3.3.2.2)
  • Recommended Action (see section 3.3.2.3)

This section shows a questionnaire that involves answering one or two questions, depending on the answers given by the user, in order to determine the progress of the Back-Fit analysis.

Question 1: Does it reduce the risk of failure (Pf × Sf) to an acceptable level?

  • Response: YES
    • Recommendation: No Change, go to step 7: Improve Task
  • Response: NO
    • Recommendation: Continue analysis, go to next question

Question 2: Can the Maintenance Action be improved to form an effective task?

  • Response: NO (Q1), YES (Q2)
    • Recommendation: Modify Task, go to step 8: Proposed Maintenance
  • Response: NO (Q1), NO (Q2)
    • Recommendation: Redesign maintainable item, go to next failure mode

3.3.7 Improve Task

"7. Improve Task" is the seventh page in the NAVSEA Road Map tab and coincides with the seventh step ("Can the Task Be Improved?") in the Effectiveness filter in the NAVSEA Road Map. This page is enabled when an effective maintenance action has been verified in the previous page.

This page contains a list of maintenance action improvements based on options provided by the road map. Each option is briefly described, and the user selects the relevant radio button for the appropriate improvement, includes a maintenance action change narrative, then selects the bottom link to the last page.
