4. Criticality Reports

MADE contains a list of FMECA and FMEA report templates that take criticality information from different criticality factors in the system model.

Generating a FMECA/FMEA report from the Report Wizard

  1. Select Reports > Report Wizard from the main menu
  2. Select a Report Type (PDF or Excel)
  3. Select a report template from the Criticality or Failure Mode Effects Analysis categories
  4. Select Next to continue to the next page

The second page of the report wizard for FMECA/FMEA report formats shows the End Effect Item, Propagation Method, and Output Options.

4.1 End Effect Item Section

The End Effect Item section consists of a tree diagram of the system model. The user can select the end effect (denoted by a dedicated icon) for the report by selecting an item and selecting the Set End Effect Item button.

4.2 Propagation Method Section

The propagation method section enables the user to select the type of failure pathing simulation used to generate the FMECA/FMEA. The user can choose between FCM or Bond simulation — for more information about these failure pathing simulations, refer to the Bond & FCM Modelling comparison guide.

4.3 Output Options Section

This section provides several options to simplify or extend the FMECA/FMEA report.

Ignore Injected Failures Does not include items with functional failures (failure diagram override) in the FMECA/FMEA report.
Include Nominal Paths Includes failure paths leading to a nominal response at the end effect.
Root Causes Only Excludes subsystems with component failures in lower LOI.
System Failure Only Includes failure paths leading to a system or end effect failure.

4.4 Report Body Customization Page

This page contains formatting fields and options for selecting page size, page orientation, report cover page footer messages, and report body footer messages.

4.5 Cover Page — Organizational Details Page

This page contains formatting fields and options for entering organizational name, addresses, numbers, and website details to be displayed in the report cover page.

4.6 Export Report Page

This final page allows the user to select the "Export to PDF" checkbox and specify the file path where the PDF version of the report shall be saved. Selecting Browse opens a dialog to specify the file path, and selecting Finish closes the Report Wizard and generates the report.

4.7 FMEA & FMECA Report Sections

Once the FMEA/FMECA report has been generated, the user can navigate and view pages and sections within the report. For the Excel report format, the user can select indentured tabs to navigate through the report pages.

The PDF FMEA/FMECA reports generated in MADE are divided into several sections:

  • Cover Page
  • Glossary of Terms
  • Criticality Definitions & Tables
  • Report Body — this is where the bulk of the failure path information is listed
