Step 6: Complete the Mapping to Import FMEA Information
(Continuing the worked example from import-configuration-guide-fmea-example-item-mapping.md, using the same source spreadsheet with items and failure mode information in one sheet.)
3.2.2.1 Step 6a: Fill in the Name of the Worksheet of the XLSX File From Which the FMEA Data Will Be Imported
The purpose of filling in the name of the Worksheet is for MADE to know from which worksheet the FMEA data will be pulled. This field is case sensitive, so the user needs to write the worksheet name exactly as it appears in the XLSX file.
To complete: Select FMECA tab, enter Worksheet name (Figure 15 shows Worksheet field, initially with an error "There are missing mandatory parameters in the fmea data import configuration").
3.2.2.2 Step 6b: Set the Number of Header Rows in the XLSX File
The number entered into the Header Rows is the number of rows MADE will skip when reading in data from the XLSX file. For this example, Header rows is 6.

3.2.2.3.1 Mapping Rules
- Enter the column header name from the source file in the Name column. This is optional, but if entered, it must match with the respective column header name in the source file.
- Properties (Item Number, Function, & Failure Mode) must be mapped.
- If the source file has multiple Detection Methods defined in one cell for a failure mode, the software considers ',' as the default separator, importing different methods as separate entries. This approach also applies to Compensating Provisions, Prevention Controls, and Substantiation Actions.
- Users can change the separator by clicking in the corresponding cell in the Configuration column, then clicking on Custom to open a dialog and define the separator used in the source file.
Figure 16 ("Ability to define Custom separator") shows the "Detection Methods configuration" dialog: "Configure the separator and splitting behavior of the List parameter." Separator radio options: Comma (default), New line, Custom (with text field); Trim values: Yes / No. Resulting Configuration column value shown as "Separator: ',', Trim: Yes".
To create a mapping (Figure 17): Click plus to add a row, enter Name (optional), select MADE property. Example: Column A → Name = "ID number" → MADE Property = "Item Number" → Type = Text → Validation = N/A.
The next two columns in the source file are Item Name and LCN (columns B & C) — item properties already mapped in the Item tab. Therefore it is necessary to define these properties in the FMEA Data tab and instruct the software to ignore them for FMEA mapping:
Figure 18 shows: A → ID number → Item Number → Text; B → Item Name → Ignored → N/A; C → LCN → Ignored → N/A.
The next two columns in the source file are Item Function and Failure Modes (columns D & E):
Figure 19 shows: D → Item Function → Function → Text; E → Failure Modes → Failure Mode → Text.
Continue the process to map all the columns in the source file. Figure 20 ("Complete mapping of FMEA information") shows the final full mapping for all columns A through AI:

This complete worked mapping table is the single most reusable artifact in this guide for anyone building a new FMEA import configuration: it shows, column-by-column, how a real-world FMECA spreadsheet's headers (item number/LCN, nomenclature, function, failure mode, local/next/end effects, detection method, compensating provisions, severity classification, failure rate, failure effect probability, operating time, failure mode/item criticality numbers) map onto MADE's internal FMEA property names.
