Origins of Failure

This chart type displays the estimated relative contributions of causes

originating from each individual component in the system that develop into failure

modes.

Relative contributions are determined by comparing the number of failure paths

that lead from each cause to the nominated failure mode(s). All paths are assumed

to have equal weighting in terms of probability.

Figure 1: Origins of failure chart shows breakdown of failure mode

origins for a system. Each bar represents the relative contribution of an

individual item, while the cumulative line displays the summation of failure

modes from each previous item starting from the highest percentile contribution.
